Stockport Family
Stockport Family is a brilliant team set up by Stockport Council that brings together all the key professionals who support children and families. Instead of schools, health visitors, school nurses, and social workers operating separately, they all work together as one joined-up team.
They support children and young people from before they are born all the way up to age 19 (or up to age 25 for young people with special educational needs and disabilities).

We want to ensure all our families are aware of the Stockport Family Portal, a secure online platform provided by Stockport Council. It is designed to make managing your child’s local services and support as straightforward as possible.
The portal is a fantastic tool for parents and carers, allowing you to:
Track SEND Support: Securely view the real-time progress of Education, Health and Care Plan (EHCP) assessments and message your allocated coordinator directly.
Access Local Services: Find information on childcare, early years support, and local family activities.
Stay Connected: Access key documents and advice safely in one central place.

Early Help Assessment
An Early Help Assessment (EHA) is the tool used to identify emerging needs for children and their families, and coordinate a plan of support to meet these needs. An EHA aims to prevent problems from becoming worse, and to provide support to stop things escalating to crisis.
